Relevant’s care management platform is used by dozens of non-profit organizations, primarily within the New York State Medicaid Health Homes program, to provide care to tens of thousands of patients with complex needs. In addition to software, we provide administrative and billing services to support our Health Home customers.
We’re hiring a Director of Health Home Services to provide strategic direction for our Health Home program, lead the delivery of Health Home billing and administrative services, and contribute to the ongoing improvement of our care management platform.
Main Responsibilities

Manage all aspects of engagement with our Health Home customers, including conducting stakeholder meetings, drafting regular written communications, and developing and implementing policies and procedures that align our services with program requirements and customer needs.

Keep abreast of the changing regulatory landscape that impacts the Health Home program and Delivery System Reform initiatives, and develop creative proposals to integrate the demands of new requirements into our offerings.

Develop and implement plans to bring Relevant’s expertise in care management software, data integration, and analytics to additional Health Homes and to DSRIP and PPS projects.

Oversee the preparation of Health Home claim submissions and customer statements, and identify ways to streamline these processes.

Become an expert user of our software platform, testing new features, providing training for customers, and proposing improvements based on user feedback, market research you’ll conduct, and insight into care management best practices.
Key requirements

Demonstrated expertise in the New York State Medicaid landscape.
Mastering the intricacies of the New York State Medicaid landscape—both its policies and its politics—involves a significant learning curve. The ideal candidate will be able to hit the ground running based on several years of previous experience in this world
and a strong professional network among stakeholders. -
Ability to manage complex projects involving multiple stakeholders.
Leading this program involves working closely with many stakeholders: our customers’ administrative staff; end users of our software platform; Health Home program officers; managed care organizations; and Relevant’s software developers and leadership team. We’re looking for someone with the ability to build strong relationships across such constituencies, and prefer candidates with demonstrated experience managing projects of similar complexity. -
Roll-up-your-sleeves effectiveness.
This is a director-level position, encompassing broad responsibility for our Health Home offerings. Working at a startup, however, means there’s rarely someone else to delegate tasks to (particularly in the first six months of this position). A typical day might therefore include moving from strategy work to troubleshooting claim denials. We’re looking for candidates who welcome such a diversity of challenges and are not allergic to scutwork. -
Superior communication and research skills.
You should excel at research and analytical tasks; be comfortable writing polished client-facing emails, blog posts, memoranda, and documentation; and be an effective speaker and facilitator in group settings. -
Tech-savvy.
We’re not looking for someone who writes code, but we are looking for someone who is clever with modern technology, cares deeply about the usability of software, and can provide insightful feedback from the user’s point of view about our platform. Excel wizardry is not required, but is strongly preferred. -
5+ years relevant work experience.
Applicants with experience in the New York State Medicaid world will receive preferential consideration. Other strong applicants may come from a variety of backgrounds, with previous experience in care management, healthcare administration or billing, program implementation, or software product management.
