Program Manager, Health Home Services

Relevant makes care coordination software which is used to care for patients with complex needs, primarily within New York State’s Medicaid Health Homes program. Our software platform is used by hundreds of care managers, at dozens of non-profit organizations, to care for tens of thousands of patients every day. In addition to software, we provide complementary administrative and billing services to support our Health Home customers.

We’re looking for someone to lead the delivery of these services to our Health Home customers, and to contribute to the ongoing improvement of our software platform.

Main Responsibilities

  • Identify emerging Health Home program needs by meeting regularly with our customers and keeping abreast of the changing regulatory landscape that impacts Health Home operations.

  • Prepare patient claim submissions and customer statements on a regular basis, and identify ways to streamline these processes.

  • Become an expert user of our software platform, testing new features and providing training and support for customers.

  • Propose ideas for improvements and new features for our software platform, based on your own ideas and by collecting feedback from users.

  • Write emails, blog posts, and documentation to update customers on the services we’re providing and improvements to the software platform.

Key requirements

  • Ability to manage complex projects involving multiple stakeholders.
    Managing this program involves working closely with many stakeholders: our customers’ administrative staff; end users of our software platform; New York State program officers; managed care organizations; and Relevant’s software developers and leadership team. We’re looking for someone who can build strong working relationships with all of these constituencies, and prefer candidates with demonstrated experience managing projects of similar complexity.

  • Superior oral and written communication skills.
    A key facet of this job involves communicating clearly and effectively to a variety of stakeholders. We’re looking for candidates who would be comfortable writing excellent client-facing emails, blog posts, and documentation regarding our services and software.

  • Tech-savvy.
    We’re not looking for someone who writes code, but we are looking for someone who is clever with modern technology. Maybe you’re an Excel wizard; if not, you’re confident you can become one.

  • Excellent research skills.
    If you started this job, would your inclination be to read everything available on the internet about Health Homes, and keep a running list of questions about how it all works? Does wading through arcane program manuals strike you as something that might be strangely appealing? Great, we want to hear from you!

  • 3+ years relevant work experience.
    Strong applicants may come from a variety of backgrounds, with previous experience in healthcare administration, program implementation, or software product management. We’ll also consider exceptional candidates from further afield.
